Medical screening needed for relocating Airmen, families

Airmen and their family members preparing to move overseas, or those going to new stateside assignments who have dependents with special needs, should be aware of some pre-move requirements.

Prior to a permanent change of station, Airmen should complete the medical screener -- AF Form 4380 - to let the 377th Medical Group know if a medical clearance is needed. Outbound Assignments will coordinate with the 377th Medical Group's Exceptional Family Member Program office to ensure the appropriate clearances actions are completed.

Medical screenings at the time of a PCS are required for Q-coded dependents and all dependents traveling to a base outside the continental United States. The purpose of the clearances is to ensure that the dependent's medical needs are able to be met at the gaining duty station. For the medical clearances, each dependent listed needs to have the following forms completed for the clearance:
- DD Form 2792 (Medical History)
- DD Form 2792-1 (Educational history for children 3-19. If your child has an Individualized Family Service Plan or Individualized Education Plan, a copy will be needed.)
- AF Form 1466-D (Dental screening for dependents over the age of 2)
- AF Form 1466 filled out by the sponsor (one per sponsor)

The forms are available online or through 377th MDG outpatient records. After all paperwork is completed, service members should contact the family medical record clearance coordinator to submit, who will coordinate medical screening appointments for all dependents traveling OCONUS or enrolled in the EFMP. Paperwork must be submitted three days prior to the scheduled appointment.

"Be proactive," said Maj. Lauren Wolf, 377th MDG's chief of the medical staff. "We highly recommend you complete the medical clearance process as soon as you receive your RIP."

She said the process can take up to three weeks for initial clearances, when it is required to send to the gaining base.

"If you know your dependents will require medical clearances, the paperwork can be completed up to six months prior to your return no later than date," she said.
